Checklist
In the packet:
1. Original Transcript or unofficial copy (and translation if its not in english)
2. Unofficial degree certificate (translation) 3. A cover letter which has details such as ur ID, full name, department name etc (optional).

Online:
1.SOP 2.LOR (can also be sent by mail if not done online)
  * And ofcourse ur scores.

Thats all. All other things mentioned here are optional and especially bank statement and affidavit are not at all required now!!

And yes there is a checklist after u complete the application form and after u get NETID.

checklist contains:
1. SCORES
2. degree certificate
3. transcript
4. application fees and Intl applicant fees
and nothing else.

Until u complete these, ur application status will remain "incomplete"
